MASTER PLAN FOR AGING PUBLIC SURVEY RELEASED
The Master Plan for Aging (MPA) Public Survey is now available electronically! To complete, please visit www.ny.gov/mpa or click here.
Input from this survey will play a pivotal role in gathering essential insights from individuals across New York State regarding their priorities for the Master Plan for Aging. Completing the survey is expected to take approximately 10 minutes of one’s time. The survey has been made accessible in a total of 17 languages, accessible through a dropdown menu situated in the top-right corner of the webpage. Very soon, a paper-based alternative, also available in these 17languages, will be available in the near future and posted online in a printable PDF format for distribution to networks. For those interested in receiving a paper version of the survey in the future, please do not hesitate to reach out to us at mpa@health.ny.gov, specifying your preferred language
The deadline for completing the survey is set for December 31st to allow the MPA team the opportunity to meticulously analyze the collected data and subsequently share it with Sub committees and Workgroups to guide their policy proposals.

USER TIPS & PLATFORM UPDATES
Be sure to either close out or send on any referrals in your “Rejected” or “Recalled” folders on the platform. Doing so helps ensure that community members are getting the help they requested.
Enter all demographic information in the Client Profile to optimize network health. Our technology platform, Unite Us, has made enhancements. For the most current updates, click on the “Chat” icon in the bottom right corner of the screen and select “News.”
NEW NETWORK PROVIDER
Conifer Park Outpatient Clinic – Plattsburgh serving Clinton, Essex, and Franklin counties. Services include both gender-specific, dual-diagnosis, and Medication assisted treatment including Methadone, psychiatric services to facilitate a well-integrated recovery program for clients, and provides a continuum of treatment for clients making the transition home from inpatient treatment or who are seeking support on an as-needed basis. Other services include early intervention and recovery topics, relapse prevention, PAWS/Stress, anxiety and meditation, self-help and sober support networks, family systems, gender specific groups for men and women, MAT groups, anger management groups, co-dependency group, dualdiagnosis groups, DWI evaluations, DWI specific groups.

Cancer: Thriving & Surviving is an evidence based self management telephonic workshop, designed for adults and caregivers impacted by cancer – no cost to attend!Runs Friday’s through November 10, 2023. See attached flyer for dates and registration information.
Virtual Diabetes Self Management Workshop will teach you a variety of strategies to help you improve your health and well-being at no cost! Sessions are Wednesday’s from 1:30pm – 4:00pm through November 8th. See attached flyer for additional information and registration details.
WANT TO LEARN MORE PLATFORM TIPS AND TRICKS?
We invite you to join our recurring Skills Development training sessions, where you’ll have an opportunity to take a deeper dive into the keys to sending successful referrals, effective referral management, and our advanced skills class focusing on SDOH screenings, internal cases, and rejected and recalled referrals.
We encourage users to attend all three sessions, regardless of network participation, to gain a greater understanding of the Unite Us platform and how to best navigate the network! Sessions are available the last Tuesday of every month from 12 pm- 1 pm.
