AN IMPORTANT REMINDER FROM YOUR REFERRAL NAVIGATOR, KATY KILLER!
If you are receiving “In-Review” notifications for referrals, it may be because some referrals sent to the Coordination Center had to be recreated and cannot be closed out. You can either turn off notifications specific to “In-Review” reminders, or look at referral notes for updates.
If you are unable to update a community member’s address in their profile, please add their current address in the referral description.
Be sure to capture all demographic information and update regularly in the community member’s face sheet profile tab.

We invite you to join our recurring Skills Development training sessions, where you’ll have an opportunity to take a deeper dive into the keys to sending successful referrals, effective referral management, and our advanced skills class focusing on SDOH screenings, internal cases, and rejected and recalled referrals.
We encourage users to attend all three sessions, regardless of network participation, to gain a greater understanding of the Unite Us platform and how to best navigate the network!
