At a time when good news seems all too rare, the Adirondack Health Institute and Unite Us are excited to share some exciting updates about the ADK Wellness Connections network!

ADK Wellness Connections will now be part of the Unite New York network, and will be expanding to cover an additional 6-county region, led by Central New York Care Collaborative (CNYCC, or CNYCares)!

As you know, ADK Wellness Connections currently covers a 9-county region (Clinton, Essex, Franklin, Fulton, Hamilton, Saratoga, St. Lawrence, Warren, and Washington counties) and is connected in the Unite Us platform to the Healthy Together network, lead by Alliance for Better Health. Healthy Together covers a 6-county region (Albany, Fulton, Montgomery, Rensselaer, Saratoga, and Schenectady counties), for a 13-county service area. Starting today, the CNYCares Referral Network will be added to the existing network, launching 50 organizations in Cayuga, Lewis, Madison, Oneida, Onondoga, and Oswego counties. Please give a warm welcome to the CNYCares Referral Network!

What does this mean for you? As a provider in the network, there isn’t anything you need to do. This just simply means you will now have access to more resources in a larger geographic footprint (19 counties) and can send referrals to providers in the CNYCares Referral Network if you have clients that live in that region (Cayuga, Lewis, Madison, Oneida, Onondoga, and Oswego counties)!

In addition,

New Service Types

Unite Us has added several new service types to the network that are now available for your use:

Individual & Family Support:

  • Family Support Home Visiting Programs
  • Developmental Delay and Disability Support

Housing & Shelter

  • Homeless Drop-In Services

Legal

  • Conflict Resolution & Mediation

If you would like to add one or more of these service types to the programs you receive referrals for in ADK Wellness Connections, please submit the Organization Update Form or email me directly.
