My name is Katy Miller; it’s nice to meet you! I am the Referral Navigator for the Adirondack region from Healthy Alliance, and locally based in Schroon Lake. I help to support our friends, Victoria Knierim and Rebecca Bigelow, at Adirondack Health Institute. We are part of a collaborative network called ADK Wellness Connections (ADKWC). Together we serve Clinton, Essex, Franklin, Fulton, Hamilton, Saratoga, St. Lawrence, Warren, and Washington Counties.

Through ADKWC, we address social determinants of health by connecting community members to healthcare and social resources that are vital to achieving positive health outcomes, while also identifying gaps in our local health and social care landscape. This isn’t my first Unite Us rodeo! I have a background in Health Home Care Management, where I managed an intensive caseload and often experienced time shortages throughout the work week (am I right?). When I started sending referrals to the Unite NY Referral Coordination Center, I found it to be a huge time-saver in the ever-changing trajectory of my day as a Care Manager. It puts the responsibility on the Referral Navigator to identify possible resources that would meet the needs of the individual seeking services – a theoretical passing of the baton, if you will. Please pass me that baton! I know how challenging workloads are and how pressed for time everyone is, so I wanted to remind you that I’m here to help minimize referral follow-up dead ends and unanswered voicemails. As a seasoned Unite Us user, I’m very familiar with navigating the platform and equally knowledgeable of the resources available in our communities.
