Benefits of Joining ADK Wellness Connections

Sep 28, 2018 | Network News

We all know that the social determinants of health (SDOH) are extremely important. Population health management studies across the country have demonstrated that addressing SDOH improves health care outcomes and substantially reduces health care costs. Research findings cite an additional 10 percent reduction in health care costs when comparing people who were connected to social services to a control group of people who were not.1 Former NYS Medicaid Director Jason Helgerson said that addressing the SDOH is “as much a moral imperative as a fiscal one.”

Adirondack Health Institute (AHI) has been committed to addressing the SDOH for more than thirty years and is proud to sponsor the new ADK Wellness Connections network. We want to thank our partners who have helped us launch the network and encourage additional partners to join us. Here are just a few of the benefits of becoming an ADK Wellness Connections partner:

  • Expand the reach of your organization’s services
  • Reduce time spent on making and receiving referrals
  • Access information about a patient or community member at-a-glance, including status updates as services are provided
  • Network with other health care and social services providers within your region and share “best practices” across regions
  • Track outcomes measurement through automated reporting

We’re committed to improving community health outcomes, especially among Medicaid populations who are often challenged by social determinants of health. We hope that you’ll join us!
