Benefits of Joining ADK Wellness Connections
We all know that the social determinants of health (SDOH) are extremely important. Population health management studies across the country have demonstrated that addressing SDOH improves health care outcomes and substantially reduces health care costs. Research findings cite an additional 10 percent reduction in health care costs when comparing people who were connected to social services to a control group of people who were not.1 Former NYS Medicaid Director Jason Helgerson said that addressing the SDOH is “as much a moral imperative as a fiscal one.”
Adirondack Health Institute (AHI) has been committed to addressing the SDOH for more than thirty years and is proud to sponsor the new ADK Wellness Connections network.
