2023 NORTH COUNTRY LEADERSHIP SUMMIT

 NEW PROVIDER: GET HEALTHY NORTH COUNTRY

Get Healthy North Country is presented by the North Country Chronic Disease Prevention Coalition. Participants offer a variety of evidence-based chronic disease prevention and selfmanagement programs designed to help individuals live their healthiest lives. Offerings include the National Diabetes Prevention Program, Chronic Disease Self-Management Program, Diabetes Self-Management Program, Chronic Pain Self-Management Program, and Cancer Thriving and Surviving.

Chronic Disease Self-Management Programs: These are six-week programs for people with chronic health problems, including diabetes, pain, cancer and/or other diagnoses that can benefit from improved self-management. The programs address common symptoms and worries from all types of chronic disease and illnesses. A NY Connects staff person will help you pick the program that makes the most sense for you.

National Diabetes Prevention Program: A year-long evidence-based lifestyle change program designed for those who are pre-diabetic or at risk for developing type 2 diabetes.

Unite Us, the technology that powers our platform, has made updates. For the most current updates, click the “Chat” icon in the bottom right corner of the screen on the platform and click “News.”

We invite you to join our recurring Skills Development training sessions, where you’ll have an opportunity to take a deeper dive into the keys to sending successful referrals, effective referral management, and our advanced skills class focusing on SDOH screenings, internal cases, and rejected and recalled referrals.

We encourage users to attend all three sessions, regardless of network participation, to gain a greater understanding of the Unite Us platform and how to best navigate the network! Sessions are available the last Tuesday of every month from 12 pm- 1 pm.
