ADK Wellness Connections Update
Dear Colleague:
AHI would like to update ADK Wellness Connections providers on some changes to the platform. As you may know, AHI sponsors ADK Wellness Connections, a free referral network powered by Unite Us, that connects individuals to a wide range of area resources.
Starting January 1, AHI will be partnering with the Alliance for Better Health which runs the Healthy Together network, covering Albany, Fulton, Montgomery, Rensselaer, and Saratoga counties. Both networks have been integrated from the start using Unite Us technology. In a change, the Healthy Together network will now be taking over managing referrals in-house, acting as the main Coordination Center within AHI’s nine-county region (Clinton, Essex, Franklin, Fulton, Hamilton, Saratoga, St. Lawrence, Warren, and Washington) in addition to their current five counties, two of which overlap (Fulton and Saratoga).
We would like to extend a big thanks to the St. Lawrence County Health Initiative, Family Service Association of Glens Falls, and Behavioral Health Services North, the organizations that have been the program’s Coordination Centers for more than two years. This was an initiative subcontracted out through the Delivery System Reform Incentive Payment (DSRIP) program, which ended on March 31 of 2020. We are happy to have been able to continue working with these organization through the remainder of 2020.
During this time, we have worked very closely with the Coordination Centers and can’t thank them enough for their fine work. We are grateful for the supportive role they have played in connecting providers to the network and community members to so many services — we couldn’t have reached this level of growth and success without them! Although we are sad to see them transition out of this role, we are excited about this new opportunity with the Alliance for Better Health and feel this model will be efficient and the most sustainable course moving forward.
Our AHI team assures you we will continue to play a supportive role for our ADK Wellness Connections network partners, sustaining the network standards you have come to expect with this referral platform.
