ADK Wellness Connections Collaborating with Healthy Together Network
One of our neighboring Performing Provider Systems (PPS) involved with New York State’s Delivery System Reform Incentive Payment Program (DRSIP) also selected Unite Us as their care coordination and outcomes tracking software vendor. The Alliance for Better Health, based in Troy, New York, launched its Healthy Together Network in March 2018 and has been working in its communities to address the social determinants of health.
The early results were recently featured in Healthcare IT News: “42 percent of all individuals in the network seeking services received housing support; 17 percent received individual and family support; 10 percent received utilities assistance; and 10 percent received clothing and household goods.”1
Keshana Owens-Cody, director of community relations at the Alliance for Better Health, said “IT is the backbone of this solution – the closed loop and easy interface make this tool not only easy for our partners but incredibly valuable…Now, the community can work together to see the whole patient and coordinate care around the success of their overall health.”
ADK Wellness Connections is proud to be collaborating with the innovative Healthy Together Network. “The collaboration between the two networks on the industry-leading Unite Us platform will create a more robust network and reduce barriers for network partners and community members in overlapping geographic areas,” said Jessica Chanese, community engagement manager at Adirondack Health Institute, the sponsoring organization of ADK Wellness Connections.
